Due to changes in the GP Contract, groups of practices are now working together - typically covering between 30-50 thousand patients in an area.
Primary Care Networks (PCNs) build on a core of current primary care services and enable greater provison of proactive, personalised, co-ordinated and more integrated health and social care services.
Primary Care Networks provide the basis for future collaboration with other providers (eg Community Trusts) where appropriate, and will work together to deliver specific packages of care.
The Clinical Director of Beverley PCN is Dr Colin Garnham.
If you are registered with one of these practices, you are part of this network. Here is a list of the practices that form the Beverley PCN:
Greengates Medical Group,
Manor Road Surgery,
North Beverley Medical Centre,
The Old Fire Station Surgery
The profiles are intended to help local government and health services make plans to improve the health of their local population and reduce health inequalities.
The Local Authority Health Profiles provide an overview of health for each local authority in England. They pull together existing information in one place and contain data on a range of indicators for local populations, highlighting issues that can affect health in each locality.
Locally a Health Profile for each of the seven Primary Care Networks across the East Riding have been produced by the Public Health team at East Riding of Yorkshire Local Authority.
